Rekürren Primer Spontan Pnömotorakslara Videotorakoskopik (VATS) Yaklaşım. Küçük Bül ve Bleplerde Stapler Gerekli mi?
Örki A, Tezel Ç, Ürek Ş, Koşar A, Kıral H, Dudu C, Arman B

Intragastric balloon for treatment-resistant obesity: safety, tolerance, and efficacy of 1-year balloon treatment followed by a 1-year balloon-free follow-up - Gastrointestinal Endoscopy
Conclusions: For patients with treatment-resistant obesity, the intragastric balloon appeared to be safe but was not a treatment option in a fifth of patients. Although an independent benefit of balloon treatment beyond diet, exercise, and behavioral therapy could not be demonstrated in the first 3 months, balloon treatment for 1 year resulted in substantial weight loss, the greater part of which was maintained during the balloon-free second year..

Intratracheal pulmonary ventilation improves gas exchange during laparoscopy in a pediatric lung injury model - Journal of Pediatric Surgery
Conclusions: Compared with conventional ventilation, ITPV improves both CO2 removal and oxygenation during CO2 pneumoperitoneum in a pediatric lung injury model. Intratracheal pulmonary ventilation may be a safer intraoperative mode of ventilation for neonates and children with respiratory failure who require laparoscopy...

The efficacy of fibrin sealant in prevention of anastomotic leak after laparoscopic gastric bypass - Journal of Surgical Research
Conclusions: An anastomotic leak model was developed in swine with disruption of the stapled gastrojejunostomy to a 12-F opening. The use of fibrin sealant significantly reduces leak and abscess complication. Our results support the tissue sealing property of fibrin sealant and its use on high-risk gastrointestinal anastomosis...

Endoscopic Ultrasonography - Endoscopy
The studies presented deal either with the outcome of EUS - i. e., its real importance in affecting the choices physicians are faced with in their clinical work - or definitively clarify certain technical issues that have often been a matter of debate at EUS meetings. Finally, a few papers describing new potential diagnostic or interventional applications of EUS are presented. Many other relevant papers in addition to those discussed in detail are cited in the references..

Successful Endoscopic Management of Iatrogenic Mediastinal Infection and Subsequent Esophagomediastinal Fistula, Following Endosonographically Guided Fine-Needle Aspiration Biopsy - Endoscopy
Complications following endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) biopsy are rare. A 75-year-old man underwent EUS-FNA biopsy of an enlarged mediastinal lymph, which histologic investigation revealed to be a metastasis of a hepatocellular carcinoma. The patient developed the postinterventional complication of suppurative infection within the mediastinum...

Patient factors influencing the completion rate in colonoscopy - Digestive and Liver Diseases
Conclusions: The completion rate was lower than that has been reported from previous hospital-based studies. Lower completion rates were found in women, older patients and in patients with diverticulosis. Higher completion rates were noted in patients with previous colonic surgery and long-standing colitis. The completion rates were not influenced by the development of the endoscopes...

Endoscopic ligation in the treatment of variceal bleeding - Techniques in Gastrointestinal Endoscopy
Bleeding from esophageal varices is a common complication of cirrhosis associated with significant mortality. Endoscopic screening is recommended in selected patients with selected risk factors. Over the last 20 years, EVL using the described technique has evolved into a readily available, effective and safe treatment for esophageal varices. Once varices are confirmed, prophylactic therapy with EVL, medical therapy, or both is warranted, which may reduce both bleeding and mortality..

Variceal bleeding refractory to endoscopic management: Indications and use of balloon tamponade, interventional radiology, and surgical treatment - Techniques in Gastrointestinal Endoscopy
Tamponade as a means of controlling esophagogastric variceal hemorrhage dates back over seventy years. Inflatable balloons of various constructs have been in widespread use since the late 1940s. Through direct mechanical pressure on communicating venous tributaries, balloon tamponade effectively cuts off variceal blood supply, with initial hemostasis achieved in up to 90% of cases. Limited by high rates of rebleeding and complications, balloon tamponade has been relegated to salvage therapy status, primarily utilized once pharmacologic and endoscopic measures fail...

Laparoscopic vs open surgical approach for intussusception requiring operative intervention - Journal of Pediatric Surgery
Laparoscopy has recently been used to treat intussusception that cannot be reduced radiologically. The effectiveness and practical nature of this approach has been questioned...Conclusions: Although there remains a group who will require a conversion to an open procedure, the laparoscopic approach should be considered a safe and effective option for all children who do not respond to a radiological reduction...

Inflammatory Bowel Disease - Endoscopy
This review summarizes important publications that have appeared during the last year dealing with imaging techniques and endoscopy, as well as the management of low-grade dysplasia and stenosis in inflammatory bowel disease. Magnetic resonance enteroclysis and capsule endoscopy are currently emerging as new imaging techniques for the small bowel in Crohn's disease...
